I am aware that kayaking can be a dangerous sport. I understand that Olympic Outdoor Center is in no way responsible for the safety of myself, or of the rented equipment. I understand that Olympic Outdoor Center makes no warranty as to the fitness or suitability of the rented equipment for the use I intend. I am fully responsible for my own safety and for the equipment. I specifically recognize there are risks involved in transporting rented equipment to and from Olympic Outdoor Center's premises. I accept any and all risks of injury and damage to the rented equipment, to property belonging to myself and others, and to myself and others personally, which risks are related to transporting the rented equipment. These risks include, but are not limited to, the rented equipment and/or racks coming untied or otherwise loose from the vehicle while in motion.

In consideration of the rental of this equipment, I hereby personally assume all risks. I further release Olympic Outdoor Center, its owners, employees, agents and operators from the liability for any harm, injury, or damage that may befall me or any equipment used by me during this rental. I further agree to save and hold harmless Olympic Outdoor Center, its agents and operators from any claims by me or my family, heirs or assigns arising out of my own participation in this rental.

I state that I am of lawful age and legally competent to sign this this affirmation and release, that I understand the terms herein are contractual and not a mere recital, and that I have signed this document as my own free act. I authorize Olympic Outdoor Center to use my photographic likeness for its lawful purposes without compensation.

I agree to replace or pay for, in a manner acceptable to Olympic Outdoor Center, any equipment rented to me that has been lost, stolen or damaged in any way while used by me. I will inspect the equipment rented to me and will not accept the equipment unless it is in good condition and I am familiar with its proper function.

I will wear a life jacket (Personal Floatation Device) of Type III at all times that I am on the water. I understand that consumption of alcoholic beverages, controlled substances, and some medications may impair my judgement and ability to paddle safely.

I realize there are inherent dangers in all watersports. I realize and acknowledge that it is my reponsibility to fully advise myself of all such dangers and that Olympic Outdoor Center is under no obligation to inform me of all the possible dangers that I may encounter.

I agree to return all equipment on time, clean, and in original working order. If the equipment is not clean, I agree to forfeit my cleaning deposit.

I have fully informed myself of the contents of this affirmation and release form by reading it before I signed it. I assume my own responsibility for my physical fitness and capability to use the rented equipment.
