INDEMNIFICATION AGREEMENT FOR MINORS
In consideration for (PRINT NAME) __________________________________ ("minor") being permitted by OLYMPIC OUTDOOR CENTER to participate in its sea kayaking instructional, touring, or event activities, I agree to this waiver, release and indemnification:
The undersigned parent of guardian of the minor, for himself or herself and on behalf of the minor, joins in the foregoing Participant Agreement, Release and Acknowledgement of risk and stipulates and agrees to save and hold harmless, indemnify and forever defend OLYMPIC OUTDOOR CENTER, as well as its agents, owners, volunteers, participants, employees, and all other persons or entities acting in any capacity on its behalf, from and against any claims, actions and demands (including those based on alleged negligence), and expenses and liabilities (including reasonable attorneys’ fees) made or brought by the minor or by anyone on behalf of the minor, as a result of the minor’s participation in activities with OLYMPIC OUTDOOR CENTER or the use of the property and facilities of OLYMPIC OUTDOOR CENTER.
I hereby represent that the minor is in good health, that there are no special problems associated with the care of the minor, and that I have adequately informed Olympic Outdoor Center personnel of any special instructions regarding the minor.
I authorize Olympic Outdoor Center personnel to render emergency first aid to the minor, and to call for medical care for the minor or to transport the minor to a medical facility or hospital (if available) if, in the opinion of such personnel, medical attention is needed for the minor. I further authorize appropriate medical personnel to render such medical treatment as is necessary for the health of the minor, in their professional opinion. I agree that once the minor is transported to the medical facility or hospital, Olympic Outdoor Center shall have no further responsibility for the minor, and I agree to pay all costs associated with such medical care and transportation.
I have fully informed myself of the contents of this Indemnification agreement by reading it in its entirety before I signed it. I have read and understand this document, and I agree to be bound by its terms. I understand that this agreement may limit my right to maintain a lawsuit against Olympic Outdoor Center in the event of injury, death or property damage to the minor, and may subject me to financial liability in the event the minor, or someone on behalf of the minor, makes a claim against Olympic Outdoor Center for injuries or damages suffered by the minor.
